What a pro does

The plumber's first split is fixture or line. A single toilet misbehaving while everything else runs clear puts the trouble in the trapway or the branch immediately behind it, and a closet auger sorts most of those out. Trouble appearing at the lowest fixture in the house, which in this housing stock is very often a basement toilet or a shower down there, points somewhere else entirely, because a basement fixture is the pressure-relief point for the whole building. Water escaping around the base is a further step again: that's flow finding the wax seal because it has nowhere better to go. On any of the second sort, the work moves to the cleanout and the main. Karst ground plays a part here too, since limestone below the topsoil shifts and settles unevenly and a lateral bedded on it can lose alignment at a joint. Resetting the toilet with a fresh seal, and checking the flange it sits on for rot or breakage, is what finishes the job properly.

Because it sits lowest in the system. When a main line blocks, waste water backs up the stack until it finds the first opening below the level of the blockage, and in a house with a basement that's the fixture down there. The upstairs bathrooms carry on draining normally right up until the level reaches them.
If water came out around the base, generally yes, since that seal has been pushed past. If the overflow stayed in the bowl and went over the rim, the seal underneath is usually undisturbed and the work stays above the floor.
In limestone country it does. Soil over karst settles unevenly as water moves through the rock beneath it, and a rigid pipe bedded in that ground can end up with a joint out of line or a dip that holds water. Neither shows any sign at all until the line starts catching solids at that point.
Not once a lower fixture has backed up, because everything from upstairs drains through the same blocked main and has to come back out at the lowest opening. Keeping water out of every drain until the line is cleared is what stops a bad afternoon becoming a much worse one.
